Blessed Gardenz provides two Medicaid-based programs that offer convenience and support to residents in paying for the assisted living services they are availing of. Both Community Care Services Program (CCSP) and the Service Options Using Resources in a Community Environment program (SOURCE) provide the same care to people who need a certain degree of assistance with daily living tasks and personal necessities. The difference lies in the income bracket of the clients who are eligible for either of the programs.
CCSP
The Community Care Services Program (CCSP) helps the elderly and individuals with functional impairments receive the care that they need with much convenience. The CCSP service options include the following services:

SOURCE
This Service Options Using Resources in a Community Environment program (SOURCE) focuses on primary medical care and supportive services. We collaborate with the patient and his/her primary care physician to discuss with all necessary medical and social services. This service ensures continuous care and minimizes the need for re-hospitalization.
